In May 2007 the Board agreed to commission an anti-Vascular Endothelial Growth Fact anti-VEGF ; service for the treatment of wet age-related macular degeneration AMD ; . The Board also agreed the use of Avastin as the anti-VEGF drug for this service. The Board expressed the wish to review the decision once guidance was published by the National Institute of Health and Clinical Excellence NICE ; in September 2007. Draft NICE guidance was released in June but following consultation NICE announced that definitive guidance would be delayed until at least March 2008. The Board's decision was reviewed again in October 2007 and following legal advice the Board confirmed its policy of commissioning an Avastin service for AMD. NICE published new draft guidance on 14th December, 2007 recommending the use of Lucentis for the treatment of AMD. This guidance is out for consultation until 14th January, 2008, and will be reviewed at an Appraisal Committee on 13th February. It is anticipated that definitive guidance will be published shortly thereafter.
In 2005 the Pharmaceuticals Division recorded its best result ever, exceeding the high, above-market growth of the previous year. Sales for the full year rose 25% in local currencies 26% in Swiss francs and 25% in US dollars ; to 27.3 billion Swiss francs, four times as fast as the global market. The gains also more than offset the decline of the Group's former top-selling medicine Rocephin following the expiry of its US patent in July. The operating profit margin before exceptional items gained 2.4 percentage points, rising from 25.0% in 2004 to 27.4% in 2005. This improvement was achieved despite higher investments in R & D, continued product launch activities and, by comparison with 2004, much lower gains from product divestments. EBITDA totalled 9 billion Swiss francs or 33.0% of sales, compared with 32.0% the previous year.

Previous studies have shown that administration of 4-AP results in the release of several neurotransmitters, including NE, 5-HT, DA, and ACh Damsma et al., 1988; Hu and Fredholm, 1991; Xiao et al., 1993; Dawson and Routledge, 1995; Versteeg et al., 1995; Schechter, 1997 ; . To determine whether enhanced levels of these neurotransmitters contribute to the DS effects of 4-AP, indirect agonists releasers and or reuptake inhibitors ; were evaluated in substitution studies. Indirect 5-HT, DA, and ACh agonists generally did not substitute for 4-AP. However, the NE reuptake inhibitor tomoxetine 50% ; , but not nisoxetine 20% ; , partially substituted for 4-AP. In addition, both imipramine 39% ; and venlafaxine 19% ; , which block both NE and 5-HT reuptake, also failed to substitute for 4-AP. Although the effects of imipramine appear to be similar to those of tomoxetine, it should be noted that imipramine very weakly substitutes only at a dose that markedly decreases response rate. Taken together, these results are not consistent with a role for NE reuptake and avc.

Both sets of granulosa cells were from women undergoing hyperstimulation, the inhibin measured in the conditioned medium might represent near maximal production capability. Furthermore, because more oocytes were retrieved from patients having relatively low serum FSH and higher granulosa cell inhibin production, dimeric inhibin may have a role in follicular development as an autocrine and or paracrine factor in promoting oocyte maturation. Thus, these data provide a potential physiological basis for day 3 serum FSH screening in ovulation induction and assisted reproductive technology programs. This study is also unique in that it is the first time a decreased concentration of dimeric inhibin has been shown to be produced by LGCs from women with declining ovarian reserve. Pellicer et al. 8 ; examined the issue of functional changes of the human ovary with chronological age using an assay for immunoreactive inhibin. They reported lower concentrations of immunoreactive a-inhibin production by cells cultured in vitro from older mean age 40 ; compared with younger women mean age 32 ; . However, the ELISA used was specific for epitopes solely on the a-subunit and thus measured both free a-subunit and dimeric inhibin. In addition, no indirect assessment of ovarian reserve day 3 serum FSH ; was described for the patients whose cells were cultured. Age-related decreases in serum inhibin have been demonstrated in regularly cycling premenopausal women 9 ; , regularly cycling infertile women 10 ; and infertile women undergoing ovarian hyperstimulation in preparation for IVF 11, 12 ; . However, day 3 serum FSH had not been examined in any of these studies. Because it is known that women of different chronological age may have similar ovarian reserve and that there is a variability in age of onset of the decline in reproductive potential associated with diminished ovarian reserve, it cannot be determined whether the patient populations studied by these investigators 8-12 ; varied with respect to ovarian reserve. Because total and dimeric inhibin elevations were of similar magnitude 1% to 2-fold increases ; , the resulting ratio of cx to dimer was similar between the low and high FSH groups. Therefore, although the capacity of the granulosa cells from the low FSH group to produce inhibin is upregulated, the pattern of a! subunit synthesis and posttranslational processing to dimeric forms does not appear different between the groups. Multiple forms of a! and dimeric inhibin have been identified in human 13 ; and bovine 14 ; follicular fluids, with varying affinities for the Australian inhibin antiserum 14 ; . Because human granulosa cell medium may contain several molecular weight forms of ol-subunit and or dimeric inhibin, the relative mass of a-subunit to inhibin dimer in the granulosa cell medium cannot be compared directly using these assays and avastin.
