If the patient indicates they have Advance Directives at home or elsewhere, a "Green Sleeve" will be initiated by the admission clerk for placement in the patient's medical record. A form will be added to the "Green Sleeve" which will notify nursing staff that family has been or should be advised to bring copies of the Advance Directive to the unit. A notice will be provided to the patient representative. Efforts will be made to follow-up with the patient representative to obtain the completed forms. If the patient is discharged prior to obtaining the Advance Directives, the form initiated at admission will be removed from the "Green Sleeve" and included in the chart as a record of staff attempts to obtain the Advance Directives. If the patient does not have Advance Directives at this time, they will be offered information and referred to the chaplain if so desired. The patient and their representative will also be provided with an address if they wish to have their advance directive filed at the hospital although they are not currently a patient. In direct contact with partially apoptotic tumor may serve as an effective means of stimulating antitumor immunity, eventually leading to tumor rejection and immune memory. In particular, this strategy may be of particular benefit in patients with surgically inaccessible brain tumors who can be treated with radiotherapy. Kikuchi and colleagues130 also recently described a similar intracranial DC vaccination strategy employing the inoculation of immature DC into partially irradiated intracranial gliomas, with encouraging results. As part of a strategy to improve DC-mediated tumor antigen presentation by means of enhancing tumor cellAPC interaction, Akasaki et al131 vaccinated DC-glioma fusion cells in intracranial glioma-bearing mice. This therapy significantly inhibited intracranial glioma growth and resulted in markedly enhanced survival in treated animals. These results were accompanied by a strong increase in specific antitumor CTL activity. Subsequently, Kikuchi and colleagues132 employed this methodology in a phase I clinical trial. Conceivably, this was not a factor in their earlier preclinical study, 131 which utilized a uniform immortalized glioma cell line not subject to the same degree of in vitro selection pressures as the primary glioma explants used in their clinical trial. In summary, the use of a treatment strategy to boost antitumor T-cell responses may prove to be the most effective immunomodulatory treatment modality for glioma. The key to the therapeutic success of this strategy will lie in eliciting an antiglioma immune response of sufficient potency and specificity in order to eliminate all residual tumor burden remaining following primary surgical resection and radiotherapy or chemotherapy. Based on the demonstrated ability of DCs to powerfully initiate specific activation of naive T cells, the use of professional APCs such as DCs to present tumor proteins in vivo may be the most appropriate way to stimulate tumoricidal T-cell responses. In addition, the identification of a panel of immunologically relevant TAAs specific for glioma may make the process more effective. However, even in the absence of clearly defined TAAs, optimizing the delivery methodology for DCs by facilitating tumor cell-APC interaction in vivo may still allow for the generation of potent antitumor T-cell responses.

These are peripheral processes, therefore we can extract asymptotic normalization coefficients ANC ; from which astrophysical S-factors and rates for radiative proton capture reactions can be precisely determined. In particular, the breakup of 8 B and 9 C is described in terms of an extended Glauber model. Existing experimental data for the breakup of 8 B and of 9 C light through heavy targets are analyzed. We show that there exists a favorable kinematical window in which breakup reactions are highly peripheral. In this regime the ANC is the better spectroscopic information to be extracted. Glauber model calculations in the eikonal approximation and the optical limit using different effective interactions give consistent, though slightly different results. This shows the possibilities and the limits of the precision of the method. Breakup reactions can be measured with radioactive beams as weak as a few particle per second, and can be used for cases where no direct measurements or other indirect methods for nuclear astrophysics can be applied. We discuss also the use of one-neutron-removal to assess n, ; rates using 15 C as example, and the proposed use of the breakup of proton drip line nucleus 23 Al to obtain the stellar reaction rate for 22 Mg p, ; 23 and dalteparin.

All professionals consider important the spiritual needs assessment, but only 71% of the members of the team do it usually. The main problems found by professionals to explore spirituality are patients in agony 85% ; , patients with difficulties in communication 71% ; and the family opposition or fears 71% ; About the evaluation methodology, 43% of professionals don't use any particular methodology and 57% professionals assess spirituality by listening and asking open questions. About the aspects that should be registered in medical records, 43% of the members would register religious beliefs and human relations and 29% not only this but also other aspects which can help other professionals. CONCLUSIONS The professionals of the interdisciplinary team are interested in giving support to spiritual needs of palliative care patients. It is necessary to unify the criteria of spiritual assessment and the way to register this evaluation in the medical records. In this way, we propose to elaborate a protocol of spiritual needs evaluation. 514. Metropolitan Jewish Health System is a highly integrated system that provides a continuum of care to chronically ill individuals of all ages. It includes three extended care facilities, hospice and palliative care services and a network of home health agencies in the New York City area. There has been an ongoing strain between the Orthodox Jewish Community and hospice and palliative care services regarding end-of-life care. Metropolitan Jewish Hospice offers an "Halachic Pathway" to every Orthodox Jewish patient and family upon admission. The Pathway gives the family an opportunity to identify a rabbi who will work with the hospice interdisciplinary team to judge all medical decisions in end-of-life care. The hospice's own Halachic advisor is available to serve this role as may a rabbi whom the family knows and trusts. Each family is guided in constructing an "Halachic Living Will" to clearly identify a unique advance directive.
