A few expensive new products for progressive disorders have provided a particular challenge. The area committee invited specialists to design pragmatic outcome studies to treat a limited number of patients in a few centres. This has been achieved by the provision by the health board of "ring fenced" sums to allow drug purchase. These studies aim to show sufficient cost effectiveness to justify subsequent funding. The drugs currently under evaluation are discussed below. Interferon alfa for hepatitis C Working with health economists, hepatologists drew up a protocol for treating hepatitis C with interferon alfa. This drug costs around 3000 per patient per annum. The economic model used estimates of clinical variables obtained from published reports and from specialist knowledge. Resource and cost variables were included, and sensitivity analysis was undertaken. Patients were recruited at two centres; some came from outside Glasgow. Strict eligibility criteria were defined. Patients are treated for 12 weeks with interferon alfa. If the virus is cleared from the plasma, a further nine months' treatment is given. Outcome data have been collected, allowing validation of the model. The drop out rate was higher than predicted, but the numbers of those who responded to treatment at three months and 12 months were approximately as expected table 2 ; . Dornase alfa for cystic fibrosis Recombinant human deoxyribonuclease dornase alfa ; was licensed in 1995 as a mucolytic for patients with cystic fibrosis. Some publicity attended the trial.

After 8 weeks of therapy, his symptoms have abated completely, his CD4 count has increased to 47 38% ; , and his viral load has declined by 3 log to 400 Test for HIV if copies mL. Samples were pooled and a real-time polymerase chain reaction PCR ; assay was used to amplify HBV DNA. Samples with detectable HBV DNA were then tested for HBV serological markers using commercially available ELISA tests for anti-HBc, anti-HBs, and HBsAg. The mean age of the patients was 35 years, 77% were men, and 50% were Caucasian. The mean CD4 cell count was 385 cells mm3, 62% had detectable HIV viral load, and only 23% were taking antiretroviral regimens, about 60% of which included drugs with anti-HBV activity. About 14% had hepatitis C virus HCV ; infection. Forty-three patients 4.7% ; were found to be positive for HBV DNA. Twelve of these 1.3% of all patients ; had occult hepatitis B with no detectable HBsAg. In this group, four tested positive for both HBV surface and core antibodies, two had detectable anti-HBc but not anti-HBs, and five were negative for all HBV blood markers. Participants with detectable HBsAg had significantly higher HBV DNA titres than those with occult hepatitis B. HBsAg-positive patients also were more likely to have elevated liver enzymes ALT and or AST ; compared with both occult hepatitis B patients and HIV-monoinfected individuals with undetectable HBV DNA. Mean CD4 cell counts did not differ significantly among the groups, and the percentages having undetectable HIV viral load were also similar. None of the patients with detectable HBsAg had hepatitis C, compared with about 14% of both occult hepatitis B patients and HBV-negative individuals. Importantly, none of the participants with occult hepatitis B were taking antiretroviral drugs with anti-HBV activity, such as 3TC Epivir ; , emtricitabine Emtriva ; , or tenofovir Viread ; . Study 2 In the second study, Vincent Lo Re, MD, and colleagues conducted a study to determine the prevalence of and risk factors for occult hepatitis B amongst HIV-positive individuals, as well as its clinical significance. The investigators analysed data from 179 HIV-infected patients with undetectable HBsAg but detectable HBV core antibodies, randomly chosen from a database of 699 study participants at University of Pennsylvania hospitals. In this study, the mean age was 47 years, 88% were men, and 75% were African-American. Onequarter had CD4 cell counts below 200 cells mm3 and 40% had HIV viral loads greater than 1, 000 copies mL. Three times as many patients were on antiretroviral regimens as compared with the previous study 73% ; , of which about 60% contained drugs with anti-HBV activity. About half of the participants 55% ; had chronic hepatitis C. In this group, 17 patients 10% ; had detectable HBV DNA using a highly sensitive transcriptionmediated amplification assay, signalling occult hepatitis B. Individuals with and without occult HBV were similar with regard to demographic characteristics. HBV surface antibodies were present in similar proportions of patients with and without occult HBV infection 41% vs 58%, respectively ; . In a univariate analysis, patients with occult hepatitis 11.

Adoption practice is principally governed by the Adoption of Children Act 1965 ACA ; , the Adoption Information Act 1990 AIA ; , some sections of the Children Care and Protection ; Act 1987 CC&PA ; , and their respective Regulations. The NSW Department of Community Services is currently preparing new adoption legislation which will repeal, replace and consolidate the Adoption of Children Act 1965 and the Adoption Information Act 1990. In addition, it is anticipated that the new Children and Young Persons Care and Protection ; Act 1998 will be proclaimed in the second half of 2000, and will replace the Children Care and Protection ; Act 1987. Parents are the legal guardians of their child, unless a Court has removed their parental responsibilities or made them joint guardians with a third party. The Director-General of the Department of Community Services becomes the child's exclusive guardian under the adoption process when all consents to the child's adoption by a parent or guardian have been given or dispensed with by the Supreme Court. Intensified peroxidation in the Graafian follicle may be a factor compromising the normal development of the oocyte. The aim of this study was to measure concentrations of three oxidative stress markers: conjugated dienes, lipid hydroperoxides and thiobarbituric acid-reactive substances, in preovulatory follicular fluids and sera of 145 women attending an in-vitro fertilization programme, and to correlate these concentrations with pregnancy outcome. Determinations were conducted either with or without an antioxidant 10 M butylated hydroxytoluene ; and an iron chelate 10 M deferoxamine mesylate ; to examine peroxidation associated with the methods used. Concentrations of conjugated dienes, lipid hydroperoxides and thiobarbituric acid-reactive substances in follicular fluid were all significantly lower than those in serum, both in the presence or absence of the antioxidant and iron chelate. These concentrations did not correlate with pregnancy outcome. In conclusion, the intensity of peroxidation in the Graafian follicle is much lower than that in serum. This gradient is the result of the lower rate of initiation of peroxidation in the follicular fluid, suggestive of the presence of efficient antioxidant defence systems in the direct milieu of the oocyte before ovulation. The concentrations of investigated oxidative stress markers in follicular fluid do not reflect the reproductive potential of oocytes. Truvada is taken as single dosage of 300 milligrams of tenofovir and 200 milligrams of emtricitabine combined into one tablet. It is taken once-daily orally. Truvada's side effects include those common to many anti-HIV drugs and more specifically to those side effects associated with tenofovir Viread ; and emtricitabine Emtriva ; . A major benefit of any combination drug is the convenience that it provides to the patient, this will translate into fewer missed doses. Thus many doctors favor prescribing combination drugs to their patients.14 Epzicom abacavir + 3TC ; GlaxoSmithKline NYSE: GSK ; Epzicom combines abacavir Ziagen ; and 3TC Epivir ; and was approved by the FDA in August 2004. It is known as Kivexa in Europe. Epzicom is a single dosage tablet combination that consists of 600 milligrams of abacavir Ziagen ; and 300 milligrams of 3TC Epivir ; . It is taken once daily. The side effects of Epzicom are those similar to abacavir and 3TC which include nausea, vomiting, fatigue, and headaches. One major drawback of Epzicom is what is referred to as a hypersensitivity reaction. This usually occurs in about 8% of people taking abacavir. If this occurs the patient must stop taking abacavir and cannot take it again because of a potentially fatal reaction.15 and entecavir!

58 One of the aims of the BisduottarPaistunturi project has been to collect and analyse the traditional knowledge of sites and means of livelihood in the area. Since the area has been and still is used by two Saami groups with their own subsistence models and totally different views of the mountain area, the task is very demanding. The starting point of the project has been to focus as much as possible on the traditional knowledge of the local people. Another task has been to combine information gathered into archives and books by earlier researchers with the actual sites in the research area. This work also includes the returning of knowledge to the society from which it has been collected. The MA-thesis of Taarna Valtonen 2003a ; deals with the river Saami cultural landscape and traditional knowledge from the point of view of place names.
