Significant damage occurred to sections of the railway embankment that carries the Conwy Valley Railway Line a bidirectional line carrying mostly passenger trains ; near to the town of Llanrwst. The damage was principally concentrated in two sections of 2.5 to 3.5 m high railway embankment approximately 1350 m and 1200 m long respectively, resulting in some areas of embankment being completely washed away, leaving the track suspended in mid-air. The repair strategy was to excavate the failed sections of earthwork and rebuild the embankment with well-graded locally sourced limestone granular fill and protect the re-built embankment with rip-rap scour.

The answer is that almost any female between puberty and menopause is susceptible to the onset of endometriosis. As younger girls in their mid-teens are being laparoscoped to investigate their period pain, we are finding an increased incidence of endometriosis in this group. Similarly, endometriosis has also been found in peri and post-menopausal women. However, endometriosis is relatively rare in this group. It would seem that endometriosis is more likely to occur in females through their twenties and into their thirties. Women who delay their childbearing would seem to be more susceptible, although young women can still develop quite severe forms of endometriosis. Stress factors may play a part and contraceptive history may be important. Pregnancy may disrupt the course of endometriosis, although not cure it in all patients. However, women who have had children are somewhat less likely to have endometriosis, although it is by means rare in patients who have had perhaps one or two children. Patients who are particularly likely to have endometriosis are those with the onset of increasing period pain where this has not been a particular problem in the past. Patients who are developing new pain on intercourse or vague, unexplained abdominal pain, which may be unrelated to either intercourse or periods, may also have endometriosis. Endometriosis can sometimes be associated with period dysfunction and irregularity. Sometimes patients may present with symptoms of pain or pressure due to endometriotic cysts in the ovaries. The classic association with endometriosis is, of course, infertility. Most patients with infertility are now laparoscoped, specifically to look for evidence of tubal damage and to check for endometriosis. Gynaecologists need to be aware that the incidence of endometriosis is increasing as more patients defer their childbearing. Therefore, young women under the age of 40 who present with infertility or abnormal gynaecological symptoms, such as pain, bleeding, dyspareunia pain on intercourse ; , or dysmenorrhoea, need active investigation to exclude the possibility of developing endometriosis.

Methods of inducing the growth of a synchronised cohort of follicles and oocyte maturation depend today on highly complex treatments with gonadotrophin-releasing hormone GnRH ; agonists and antagonists, and highly purified preparation of recombinant FSH, LH and hCG. The presence of underlying reproductive disorders, like polycystic ovary syndrome PCOS ; , in subfertile patients also demands the modification of ovarian stimulation protocols. It is clear that there are many factors that combine to determine the overall success of in-vitro fertilization treatment. Of these, the methods and drugs used for ovarian stimulation form an important part. Recent research is revealing new strategies and other pharmacological compounds that can make controlled ovarian stimulation simpler, cost-effective and more personalised for patients. However, for some of these approaches, safety, patient satisfaction and clinical efficacy in terms of live birth rates following assisted reproduction remain a matter of much debate. NSAIDs are widely used for treating symptoms of rheumatoid arthritis. They act by inhibiting the enzymes cyclo-oxygenase I and or II, which act on lipid substrates in cells, converting them to prostanoids. Tissues such as the gastric and duodenal mucosa, blood vessels, and platelets constitutively express cyclo-oxygenase I, which in the gastroduodenal mucosa regulates the production of prostaglandins, including prostaglandin E2, that exert a protective effect on its integrity by reducing acid secretion and increasing the secretion of mucus and bicarbonate. Cyclo-oxygenase I-induced prostaglandin E2 promotes platelet aggregation and its prothrombotic effects. By contrast, cyclo-oxygenase II is mainly induced in macrophages and polymorphonuclear cells at sites of inflammation by pro-inflammatory cytokines such as IL-1 and TNF-. NSAIDs that inhibit both cyclo-oxygenase I and II activity therefore compromise the gastroprotective effect of cyclo-oxygenase I, whilst simultaneously exerting a therapeutic effect by inhibiting the production of inflammatory prostanoids. Selective or specific cyclo-oxygenase II inhibitors should in theory block inflammation without gastropathic effects. The conventional and currently widely used NSAIDs are inhibitors of both cyclo-oxygenase I and II. In the United States they are responsible for admission to hospital of over 1 per cent of patients with rheumatoid arthritis per year for complications such as peptic ulceration, gastric haemorrhage, and perforation, and account for a twofold increase in death over expected rates. It is claimed that the least gastrotoxic are ibuprofen and nabumetone, with naproxen and diclofenac carrying intermediate risk, followed by drugs with a high risk such as fenoprofen, ketoprofen, indomethacin and piroxicam, and azapropazone see Table 2 for dose ranges ; . For patients who develop dyspepsia and or NSAID-induced gastropathy, or elderly patients who have a high risk of gastroduodenal sideeffects, concomitant administration of prostaglandin analogues such as misoprostol ; or proton-pump inhibitors such as omeprazole or lansoprazole ; is recommended, and NSAIDs are best avoided for patients with a history of peptic ulcers. Eradication of Helicobacter pylori infection results in long-term healing of pre-existing gastric and duodenal ulcers, but whether it decreases dyspepsia or ulceration caused by NSAIDs is uncertain. Drugs such as meloxicam and etodolac that act by selective or specific inhibition of cyclo-oxygenase II and thus spare cyclo-oxygenase I have fewer gastropathic effects. Two highly cyclo-oxygenase II selective inhibitors, celecoxib and rofecoxib, possessing no significant cyclo-oxygenase I inhibitory activity at anti-inflammatory therapeutic doses, have been recently introduced. Clinical trials have demonstrated their improved safety profile in respect to endoscopically detectable gastroduodenal ulcers, upper gastrointestinal haemorrhage, and perforation when compared with conventional NSAIDs. Although the inhibition of platelet function by NSAIDs that inhibit cyclo-oxygenase I is associated with serious gastrointestinal haemorrhage in susceptible individuals, certain of these drugs, such as low-dose aspirin, appear to be beneficial in the prevention of strokes and coronary thrombosis, a property not shared by selective cyclooxygenase II inhibitors.
