Why individuals from essentially the same social background and profession should have such differing views was not explained in an empirically verifiable way. Thus they are forced to build and foster good relations with the political elite and members of the state bureaucracy. In fact they tend to become clients of the political elite of the moment. The other group is made up of those in less senior position whose connections in this area are more distant, striving more to build up and preserve a network of contacts within their own professional circles. And yet with such firm evidence of effect and a logic connecting their mode of action to the analgesia that is often observed, many patients find their use complicated by unacceptable side effects that preclude sustained use.
