Most countries suffer from what I call the illusion of national immunit: for one reason or another, they think that drugs or AIDS have nothing to do with them because they are Christians, Buddhists or Muslims, communists or capitalists. You have stated that harm reduction is based on individualism and that the "cultural specificities" aspect of this approach raises questions as to whether it can be exported to other countries. Can you explain this idea in greater detail ? I have not yet given this matter a great deal of thought, but it is certainly interesting to note that harm reduction is more strongly supported by some countries than by others. This may be because they see human rights, individual worth and responsibility and so on in different light. In many countries, this depends on whether there exists a proper public health system, in terms of both the concept and the infrastructure. The focus is on utilizing good breathing practice, louder voice volume and wider pitch range; thus addressing some of the most serious voice symptoms found in people with PD. Participation in a Tremble Clefs singing program can help overcome these problems through breathing, stretching and posture activities, vocal exercise, rhythm and movement, and a strong social support system. The benefits are enjoyed by those both with and without previous singing experience. Research has shown that vocal exercise enhances voice volume and quality in people with Parkinson's, the presence of music and rhythm improves movement, and participation in a social group improves psychological functioning. We'll have to stick to that date. I think the prospect of the elections is making a number of spoilers nervous. What needs to change between now and the February 7 elections for the vote not to be postponed again? I think the main difficulties for the election so far have been logistical. It's been a major effort of the Organization of American States. They have put a lot of work there. There have been difficulties in producing the cards and then distributing them and making sure that everybody will collect the cards and that they will vote. So for that, you need enough time to inform the people so that they are in a position to vote on the day of the election. We are now quite convinced that there is no major technical impediment to the election. The only real threat is the people who do not want an election, because in an election, there will be winners and there will be losers. Haiti has been really afflicted by this sort of winner-take-all culture, where people feel that they can't afford to lose. Our message is, "Yes, there will be winners and losers but it will be in a fair and transparent election." We will make sure that is the way the election happens. We will also make sure the winners respect the losers and that Haiti after the election will not sideline all the people who lost election. I think that is very important and that Haiti has to move beyond that culture of winner-take-all. One of the reasons for the mission to be strong in Haiti is precisely [to help] get over that.

The aim of the present study was to examine whether essential hypertension is associated with altered plasma concentrations of dihydroxyphenylglycol, the principal presynaptic metabolite of norepinephrine. Forearm venous plasma dihydroxyphenylglycol and norepinephrine were determined at rest and during graded orthostasis in 47 normotensive control subjects and 58 outpatients with essential hypertension. There was no group difference in age. At supine rest as well as during sitting and standing, hypertensive subjects had plasma norepinephrine concentrations similar to those in normotensive control subjects, but plasma dihydroxyphenylglycol concentrations were higher than those in normotensive control subjects. Both groups snowed a linear relation between plasma dihydroxyphenylglycol ordinate ; and plasma norepinephrine abscissa ; . The resulting regression line was steeper p 0.02 ; and its ordinate intercept higher ? 0.01 ; in hypertensive than in control subjects. Eleven normotensive and 14 hypertensive subjects were also tested 3 hours after desipramine 1.5 mg kg orally ; was administered to inhibit neuronal norepinephrine reuptake. The drug did not alter plasma norepinephrine, but did reduce plasma dihydroxyphenylglycol and did abolish plasma dihydroxyphenylglycol responses to upright posture in both groups of subjects. The mean plasma dihydroxyphenylglycol concentration observed in the presence of desipramine again was higher in the hypertensive than in the control group p 0.01 ; and closely agreed, in both groups, with the dihydroxyphenylglycol concentration given by the ordinate intercept of the dihydroxyphenylglycol versus norepinephrine regression line in the absence of desipramine. Thus, the desipramine-resistant and desipramine-sensitive component of plasma dihydroxyphenylglycol i.e., dihydroxyphenylglycol formed from vesicular norepinephrine leaking out of the transmitter stores and from synaptic norepinephrine subsequent to neuronal recapture, respectively ; appear to contribute to the elevated plasma dihydroxyphenylglycol concentration in essential hypertension. Mice. Apparatus. The apparatus was a 5.5-cm-wide circular track constructed of gray Plexiglas with an inside diameter of 34 cm, a midtrack circumference of approximately 121 cm, and an elevation of 40 cm. It consisted of two open quadrants with a raised, 2-mm edge and two closed quadrants with walls 11 cm high. Because the zero maze had no central area, the animal had to be in either an open or a closed part of the arena. Experimental procedure. After oral drug administration 60 min prior to test ; , the animals were returned to their home cage. Mice were placed in one of the closed quadrants designated as the starting quadrant and were allowed to investigate the zero maze for a period of 5 min. During this time, an observer scored mice on several anxiety-related variables as identified in previous studies Shepherd et al., 1994; Tarantino et al., 2000 ; . These included time spent in both open and closed quadrants, number of transitions between quadrants, latency to leave the closed quadrant, stretchings elongated body posture with at least snout over open closed divide ; into open quadrant, rearings, and head dips. Drugs. GS39783 3, 10, and 30 mg kg p.o. ; , chlordiazepoxide 10 mg kg p.o. ; , or vehicle 0.5% methylcellulose ; were administered 60 min before the test. Rats. Apparatus. The maze was essentially a larger version of the mouse test and consisted of a gray Plexiglas annular platform 105-cm diameter, 10-cm width ; elevated to 65 cm above floor and divided equally into four quadrants. Two opposite quadrants were enclosed by gray Plexiglas walls height: 27 cm for the outer wall and 20 cm for the inner wall ; , whereas the remaining two opposite quadrants were surrounded by a small border 1 cm high ; . The apparatus was illuminated by white light 150 190 lux ; . Experimental procedure. After oral drug administration, the animals were returned to their home cage. After an appropriate pretreatment time 60 min ; , rats were individually placed into a closed section. A 5-min trial was performed, and between subjects, the maze was thoroughly cleaned with Thedra Thedra, Zwingen, Switzerland ; . Unlike the mouse test, the different parameters [the time spent in open quadrants, the distance ratio expressed as the distance traveled in open quadrants total distance traveled ; , and the total distance traveled] were automatically recorded and analyzed by means of an in-house developed video-tracking system and quantified with appropriate software. Briefly, an ordinary black and white camera mounted 2 m above the zero maze was connected to a frame grabber type DT3155; Data Translation, Inc., Marlboro, MA ; . Spasticity in a stroke patient is often characterized by tightness or increased tone of the involved extremity. The patient usually assumes a posture in which the upper extremity is adducted and internally rotated at the shoulder, flexed and pronated at the elbow, and flexed at the wrist and fingers. The lower extremities usually held with the hip internally rotated and extended, the knee hyperextended, and foot plantar flexed and supinated. Spasticity is difficult to control. Figure 2A shows results in which the IC50 values are comparable for the inhibition of CYP 1A2 by furafylline when probed with either Vivid Blue or phenacetin 1.1 and 1.5 M, respectively ; , whereas for CMPD1A221, the IC50 values are distinct with the fluorogenic and conventional probes Fig. 2B; 9.5 and 30 M, respectively ; . However, both probes do indicate the compound's ability to inhibit CYP 1A2. Figure 2, C and D, shows that ibuprofen and warfarin are inhibitors of CYP 2C9 when probed with diclofenac IC50 19.1 and 21 M, respectively ; , whereas when probed with the fluorogenic substrate MFC, ibuprofen showed no inhibition Fig. 2C ; , and warfarin was an activator Fig. 2D ; . Similarly, debrisoquin, a known CYP 2D6 substrate, is an inhibitor of CYP2D6 when probed with the fluorogenic substrate AMMC IC50 9.4 3.9 M ; , whereas with bufuralol as substrate, activation is observed Fig. 2E ; . The mean IC50 values for inhibitors examined with the fluorogenic and conventional probes are listed in Table 3. The estimated medians, fitted lines, and prediction intervals are plotted for each P450 in the correlation diagrams shown in Fig. 3, panels A through E. The actual raw data for the uncensored values were not plotted since each of the three replicates did not correspond to a specific replicate from the other probe. The fitted linear equation for each figure is given in the corresponding figure legend. Although the computed intercept for 2C9 was significantly different from zero [0.9 0.4 p 0.03 ; ], the intercept was small and the R2 value was 0.49 in the intercept model compared to 0.59 for the no intercept model. Therefore, no intercept was used for any of the P450s. As shown in Fig. 3, A through E, there are censored values on the margins of the plots top x-axis and right y-axis ; that fall outside of the prediction intervals hereafter referred to as outliers ; . For those outliers that are censored for the fluorescent probe and not for the conventional probe, it is not possible that these data would ever be within the prediction limits, even if the IC50 could be measured at its true, uncensored value. This also holds true for outliers that are censored for the conventional probe and not for the fluorescent probe. For example, in Fig. 3E, the censored fluorescent values on the right y-axis, shown by the arrow, would have true values that would have the same conventional values but a greater fluorescent value. The true value, therefore, could lie on the fitted line. On the other hand, the points indicated by the arrow on the top x-axis would have true values that would be even further outside the prediction limits. For all five P450s the censored points that could never be within the prediction limits were counted, and the results are shown in Table 4. The cell frequencies are not high enough to perform individual chi-square tests on each P450; however, the overall analysis of the totals shows no significant difference p 0.7 ; . The difference between the outlier frequencies for P450s 1A2 2C9 2C19 and 3A4 is due to the very flat response seen for 3A4 in Fig. 3E compared with the other P450s. As shown in Fig. 3A for CYP1A2, for those compounds that fall within the boundary limits, there appears to be a linear relationship R2 0.7 ; , although the variance increases with increasing IC50 values. The high slope of the line 2.4 ; indicates that the fluorogenic probe provides a lower IC50 value than the conventional probe, and it is more likely that the conventional probe readings will exceed the boundary limits and be censored. However, although the conventional censored data are within the prediction limits, the fluorescent-censored data are not, and do not obey the observed linear relationship from noncensored data. Ten of the 60 compounds tested were censored either for the fluorogenic or conventional probes. This corresponds to 17% of the total tested. The relationship observed for CYP 2C9 is similar to that for CYP.

The pigs had been gentled previously and were allowed to lie down at will. Photographs were taken only when the pigs had normal leg positions. Four Hampshire x Yorkshire pigs were used.
