Paired immunophenotype in immature and mature DC obtained from patients with HCMV mononucleosis as compared with healthy subjects. A key role among the different functions performed by DCs in controlling immunity and tolerance is the secretion of cytochemokines. This function is fundamental for specific recruitment of immune cells in the infection area and for inducing different T cell responses [36]. Here, we found that acute HCMV infection in immunocompetent individuals induced a proinflammatory pattern of cytochemokine secretion in DCs, with significantly increased production of IL-1 , TNF- , CCL2, and CCL3 and a reduction in the release of the antiinflammatory cytokine IL-10. Nevertheless, the infection alone was not enough to activate the secretion of proinflammatory cytochemokines, as the boost of inflammatory soluble factors was revealed only in DCs obtained from HCMV-infected patients following LPS stimulation. The reduced production of IL-10 by DC obtained from HCMV-infected patients indicates that this anti-inflammatory cytokine is not involved in impairing the expression levels of MHC class II molecules and in subverting the immunostimulatory properties of DC in our patients' group. Nevertheless, such viral-induced inhibition of DC function may also be caused by mechanisms other than production of IL-10, such as expression of the glycoproteins US2 and US3 [37] or secretion of a soluble form of CD83 [34] in infected cells. These alternative strategies may account for the impairment of phenotype and immunostimulatory function in DC, which we observed in patients with HCMV mononucleosis. It is at present unknown whether cmvIL-10 is produced during a natural infection and contributes to the viral-induced, impaired immunophenotype and functions of DC. Production of inflammatory cytokines, such as TNF- , in response to HCMV infection would be expected to help in combating the virus infection. However, HCMV has been shown to prevent TNF- signaling in infected cells by downregulation of one of the TNF- receptors at the cell surface [38]. In addition, our group has demonstrated previously that TNF- , together with interferon- , rather than inhibiting replication of HCMV, is responsible for the differentiation of monocytes into HCMV-permissive macrophages, which are refractory to the antiviral effect of these cytokines [39]. TNFsecretion by HCMV-infected monocytes has also been shown to inhibit T cell proliferation through arachidonic acid and prostaglandin E2 release [40]. Furthermore, an association has been reported between high TNF- plasma levels and the development of HCMV disease in transplant patients [41, 42], and an inverse correlation has been demonstrated between high IL-10 TNF- ratio values in plasma and the development of symptomatic HCMV infection [43]. Thus, the amplified, proinflammatory cytokine profile, which we detected in LPSstimulated DCs from patients with HCMV mononucleosis, may play a role in the antiviral response but may also take part in the development of symptoms i.e., fever, asthenia ; and may contribute to immunosuppression. Cell-bound receptor-ligand systems. It has become apparent that the forced expression of one or more of these agents within a tumor cell is capable of greatly enhancing the immune response to the weak tumor antigens that cell may express. The immune response so generated may then be effective elsewhere in the body against nontransduced cells. This immunologic bystander effect is an important consideration, since the inefficiency of vectors, currently available, makes the probability of transducing all tumor cells in a patient exceedingly remote. Hence, by transducing even a small proportion of cells, it may be possible to use the efficient targeting mechanisms of the immune system to ensure that the response affects the bulk of tumor cells, including those that were not genetically modified. Genetic modification of tumor cells A number of different agents have been successfully utilized in animals, including chemokines such as lymphotactin [54], agents which enhance antigen presentation such as GM-CSF [55, 56, 57], and cytokines that enhance CD4 cell activity eg, TNF and Interleukin 7 ; [58, 59], increase expression of class I MHC antigens eg, gamma Interferon ; [60], or amplify T cell responses eg, Interleukin 2 ; [61]. Additionally, efforts have been made to express costimulator molecules on tumor cells, including CD40 Ligand [62, 63, 64, 65, and B7.1 [67], or intercellular adhesion molecules such as ICAM 1 and ICAM 3 [68]. Source of cells The cells used for gene-modified tumor immunotherapy may be derived either from the patient themselves or from an allogeneic cell line grown in culture. Each of these approaches has reciprocal advantages and disadvantages. For example, an autologous cell line, unlike an allogeneic cell line, will almost certainly express the tumor specific antigens that are present elsewhere in the patient. The heterogeneity of human tumors means that this may not be so for an allogeneic tumor cell line. An autologous cell line will also express these tumor associated antigens in the context of the patient's own MHC molecules, and so will be recognized by the host immune system. An allogeneic cell line will likely only do this if the antigens on that cell line are taken up by host antigen presenting cells and subsequently presented to the host immune system cross priming ; . The clear advantages of allogeneic tumors are that they are much more readily obtained in quantity than autologous tumors, which may be difficult to harvest in adequate numbers to generate a vaccine. Allogeneic cells are also much easier to standardize, since the level of transgene expression will be constant and will not vary from patient to patient. This makes the design and interpretation of clinical trials much simpler. Finally, from a practical point of view, should a tumor vaccine be promising in early phase clinical study, the development of an allogeneic vaccine would be substantially and mitotane.
